We live and work in a technology-driven, information-focused world. My colleagues and I are highly educated and trained; we spend our days interacting with the world through laptops, smart phones, and tablets. We are knowledge workers, not clerks, and most (but surely not all) of this technology-enabled work is professional and not clerical work. Why would we think physicians and other clinicians would also not be actively engaged with digital applications throughout their day?

Dear Colleagues:
Integrating the Healthcare Enterprise (IHE) and American Health Information Management Association (AHIMA) invite you to submit comments on the White Paper entitled Patient Registration Demographic Data Capture and Exchange.

The White Paper was developed by the IHE Patient Care Coordination (PCC) Committee with participation of the AHIMA Standards Task Force. It is based on the AHIMA Patient Registration Use Case and specifies the requirements and constraints for patient demographic data that should be collected and exchanged for patient registration during an emergency department visit at a healthcare organization.

These data requirements may be proposed to be published as future IHE Technical Framework Volume 4: United States National Extension to the IHE ITI Patient Administration Management (PAM) Profile for the message-based data exchange.

Office of National Coordinator: Interoperability Standards Advisory

The Interoperability Standards Advisory (ISA) represents the model by which the Office of the National Coordinator for Health Information Technology (ONC) will coordinate the identification, assessment, and public awareness of interoperability standards and implementation specifications that can be used by the health care industry to address specific interoperability needs including, but not limited to, interoperability for clinical, public health, and research purposes. ONC encourages all stakeholders to implement and use the standards and implementation specifications identified in the ISA as applicable to the specific interoperability needs they seek to address. Furthermore, ONC encourages further pilot testing and industry experience to be sought with respect to standards and implementation specifications identified as “emerging” in the ISA. Stakeholders are encouraged to review the content online and comment to provide feedback to ensure the ISA is as accurate as possible. Visit https://www.healthit.gov/isa/ to view the latest content. Chesapeake Regional Information System (CRISP) Best Practices

In 2009, the Maryland Health Care Commission (MHCC) and the Health Services Cost Review Commission (HSCRC) designated the Chesapeake Regional Information System for our Patients (CRISP or State-Designated HIE) to build and maintain the technical infrastructure to support a statewide HIE. The long-term goal of the State-Designated HIE is to build the fundamental foundation for interoperability to communicate health data among Maryland physicians, hospitals, and other health care organizations and providers. The State-Designated HIE will also enable communities with regional HIEs to connect to other communities around the State and, in the future, with providers in other states.

Best practices

  • Wide range of data - We created a report that combines hospital claims data to allow users to filter with hospital MRN, etc. It is through this report that hospitals identify the high-risk patients. CRISP gets all hospital claims data, mpi – longitudinal records.
  • Provider access - Report goes to doctor, on the front end of care coordination. List can be exported so that doctor can follow up.
  • Analytics - The value we add is analytics that can flag readmissions, chronic conditions etc. Report gives director of care management insight intotrends and allows the function to search patient by condition. This functionality is a powerful tool that can be used in initiatives to avoid readmissions, etc.
  • Training - In our organization we built a training program so they understand use cases
  • Goal setting - We have set milestones – actual people who use the technology, number of times accessed.
