Avoiding Silos in Healthcare: Empowering Care Coordination Across Settings

Access to relevant patient data integrated with additional information from payers and other sources at the point of care can enable providers to create a more comprehensive longitudinal patient record. With a comprehensive patient record, providers can proactively identify health trends and patterns, moving away from reactive, uncoordinated care and toward more holistic patient care coordination.

ICD-10 Survey Results

With the compliance deadline for implementing the ICD-10 code set only four months away, eHealth Initiative (eHI) and the American Health Information Management Association (AHIMA) surveyed healthcare providers in spring of 2015 to understand how they are preparing for the transition, assess the anticipated impact of the transition, and explore post-implementation plans.
