Workgroup: How helehealth and analytics are being applied with SDOH to improve outcomes 

Dr. Amy Salerno, Director of Community Health and Well-Being, from the University of Virginia Health System, shares how they are enhancing transitions of care, identifying patients most at risk, and aligning the extended care team including clinicians and Community Based Organizations (CBOs) to utilize data on SDOH to intervene and have a positive impact on patients. The Community Health Collaboration platform from Pieces Technology helps healthcare organizations connect patients with community groups that address the social, economic, and behavioral factors that affect people’s health. The COVID pandemic has heightened the urgency and UVA has introduced approaches to address the situation and specific needs. We will also be joined by Ruben Amarasingham, MD, Founder and CEO of Pieces Technology.
