Validic is a technology platform for convenient, easy access to digital health data from clinical and remote-monitoring devices and applications. Their innovation had a six-month time frame and was designed to create a defined workflow for PGHD from devices and apps to the patient portal and electronic health record (EHR). This allowed patients to view their health data and vitals readings immediately that allowed them to see the health impact of their lifestyle decisions in near real time and over time. It had the goals of using remote monitoring to leverage PGHD for increasing engagement and adherence with care teams, easing care team burden for streamlined processes to capture data outside of the clinical setting as a means for risk prevention, prevent and reduce readmissions by identifying most at-risk patients and allocating additional time on their care efforts, immediate cost savings from reducing the amount of physician and clinical resources, and create a scalable, developed value-based care model within the health system.
