Elements of new models of care delivery, such as Accountable Care Organizations and Patient-Centered Medical Homes, bundled payment programs, as well as financial incentive programs like the Electronic Health Record (EHR) Incentive Program and Meaningful Use will be reviewed to identify the means and motivation behind what works in the real-world setting when it comes to the adoption and utilization of health IT with both healthcare consumers and healthcare providers.
